Incident Investigation - self-paced
Course Description:
Although we take every precaution to lower the likelihood that a safety incident will occur, accidents do happen and then an investigation is needed. Anyone may be involved in an incident investigation. If you have a management role, you may be asked to carry out an initial assessment or even lead or be part of a local investigation team.


This course will equip you with the tools that you will need to carry out those roles competently.



There are six modules in this course, consisting of four Lessons and two Practice Sessions.



  • Lesson 1: Why investigate (14 minutes).
  • Lesson 2: What is an investigation (14 minutes).
  • Lesson 3: Investigation techniques (14 minutes).
  • Lesson 4: Reporting writing (8 minutes).
  • Practice Session 1: A screw loose (10 minutes).
  • Practice Session 2: Watch your step (13 minutes).

    You will learn:

  • Why investigations are necessary.
  • How to conduct an investigation.
  • The contents of an incident report.

  • Intended Audience:
    The incident investigation course is aimed at staff who will either lead or assist in the investigation of incidents or accidents in the workplace.
