Assessment Query Form

The Assessment Query Form (AQF) should be used by all students who have a query during an assessment or suffer a technical failure while using AssessmentUCL. 

This form should only be used for assessments which are delivered via AssessmentUCL.  (Departments running assessments on other platforms, such as Moodle, should ensure that their students know how to contact them in the event of a query).  A link to the AQF should always be added to each flow (assessment instance) within AssessmentUCL, to provide students quick and easy access to it. 

We have made some behind the scenes improvements to the way in which the form works, which has necessitated replacing it.  The link to the new form is provided here:  AQF Form

The previous form is still active, but now contains a message re-directing students to the new one. 

Please ensure that the new link is used for any new assessment instances (flows) which are created in AssessmentUCL.
