New Online User Request Form

Many of you will have completed our previous ‘Data Change and Access Request Form’ we have created a new simplified version for staff and managers to request new user access to SID. 

Please note requests should be made at least 2 weeks prior to the start date to ensure access is ready. All new users will be required to attend the mandatory training in the UAT environment before access will be given to the LIVE version of the system.

Should these dates not be suitable for you, please contact SIDhelp to discuss training opportunities further. 

In addition to the New Online User Request Form, we have also created an Online Sign-Up form to help us manage who will be attending the training sessions. This replaces us sending our invites for you to forward onto your staff. Instead, your staff members will now be able to access the upcoming dates and let us know directly which session they will be attending.
