Reporting and accident or incident

Incidents that must be reported:

  • accidents
  • near-misses 
  • work-related ill-health

The UCL RiskNET tool has an online incident reporting system uses specific descriptions for incidents. Read more on their website

You should report the incident or accident if you are:

  • the individual involved
  • the individual’s line manager 
  • a witness
  • the first aider

How to report incidents

It is mandatory at UCL that all incidents are reported using the RiskNET report an incident online tool.

Fire safety procedure

Information on fire safety can be found on the UCL estates website.

Prevention

The following steps must be carried out to prevent fire:

  • All electrical equipment not in use should be switched off at the wall each evening. Doors should be closed at night and whenever a room is left empty for long periods.
  • Fire doors should not be left wedged open, their role is to prevent the rapid spread of the fire and compartmentalise corridors and large open areas.
  • Plugs that heat up, frayed cables, a slight smell of gas etc. are all early warning signs and should be reported to the UCL help desk (ext 30000) immediately, stating precise location.
  • Corridors should always be kept clear of combustible materials and other clutter.

Fire Evacuation Plan

The fire alarm signal in 222 Euston Road is loud continuous ringing.

When the alarm sounds all personnel must:

  • Stop work immediately
  • Close all windows and doors.
  • Stop all machinery, switch off electrical supplies to equipment and leave the room. The last person out should close the door.
  • Leave the building immediately by the nearest fire exit free from smoke and proceed to the fire assembly area in Euston Road/Stephenson Way.
  • Do not collect your personal belongings.
  • Do not use the lifts.

Discovering a Fire

If you discover a fire, break the nearest ‘break glass alarm’, telephone 222 and inform the operator of the building name (222 Euston Road) and the fire zone number (150). Leave the building as per evacuation procedure.  Fire fighting equipment is available on every floor, but these have been installed for the use of the fire brigade. You should make yourself familiar with the location of fire alarm points for the building.

All fire extinguishers are Red and are water-mist type and can therefore be used on all types of fire.

Fire Blankets are available in kitchens.

If any fire fighting equipment is used always notify the DSOs immediately.

On discovering a fire, sound the fire alarm.

The fire alarm is a loud continuous ringing. When it sounds:

  1. Close all windows and doors.
  2. Stop all machinery, switch off electrical supplies to equipment, turn off gas cylinders, and leave the room. The last person out should close the door.
  3. Leave the premises by the nearest escape exit or staircase free from smoke. 
  4. DO NOT use the lifts.
  5. On leaving the building, assemble in the area assigned for your building. Do not obstruct the roadway or exits.

The duty of the fire evacuation marshals (FEM) is to ensure that as far as possible, taking due regard for their own safety, that all personnel leave the building as quickly and safely as possible.

Everyone should make themselves familiar with the fire exits.  Although fire extinguishers are available it is recommended that staff do not fight fires and the first priority is to evacuate the building.

Assembly areas

If you exit from the front of the building you should turn right and assemble on the corner of Euston Road and North Gower Street.

If you exit from the rear of the building you should assemble on the corner of Stephenson Way and North Gower Street.

There should a Fire Evacuation Marshall (FEM) allocated for each floor; please make yourself aware of whom you FEMs are (see pages 5 and 6 of the safety policy).

Fire Wardens

For each floor of the Building there are two trained fire wardens.  Fire wardens are trained to evacuate areas in the event of the fire alarm. The roles and responsibilities of fire wardens can be found on the UCL estates website.
