Accidents Reporting and Investigation

Planning for emergencies before they happen is integral to risk management. This page will help you consider what you need to have in place to manage emergencies & any accidents/incidents/near-misses.

Reporting accidents, incidents and near-misses


Here is a summary of UCL policy for reporting incidents.

  • All incidents, accidents, near misses and work-related ill health must be reported.
  • All reports must be made using the riskNET 'Report an Incident' online tool. 
  • Anybody can report an incident - the individual, the individual's manager, the witness, the first aider etc.
  • All incident reports must be reviewed and signed off within 5 calendar days.

riskNET is a user-friendly, interactive online tool for reporting accidents, incidents, near-misses and hazard observations. Anyone can report an accident or incident on riskNET; as well as by those who are responsible for investigating and reviewing accidents and incidents.

How to investigate an accident/incident/near miss

We must investigate all accidents and incidents so we can: 

  • Ensure action is taken to prevent a recurrence.
  • Meet statutory requirements.
  • Help monitor and improve health and safety performance.
  • Provide information for responding to claims made against UCL.
  • Enable UCL to respond quickly and accurately to external enquiries.

Review

Departments should analyse and review their data on accidents/incidents/near-misses to identify patterns and put controls in place where possible to reduce/prevent recurrence and to share any lessons learnt with the wider UCL community.
