This research aims to understand how individual and households characteristics affect health service use and costs by setting of care for patients in their last year of life. We use linked, routine data of health and social care service utilisation across all settings of care (primary, community, acute, mental health and child and adult social care) with council data in Barking and Dagenham. This project will inform national policy on end of life care by identifying opportunities for better integration of health services for frail and older people. This project is part of the NIHR ARC North Thames.
