How to make a referral

The clinic has a national referral base and has agreed referral pathways with the local CCGs. Referrals are accepted from primary, secondary and tertiary care. 

For general practitioners referring patients, referrals must be submmitted via ERS (NHS e-referrals portal)

Other referrals can be emailed to uclh.cognitivedisordersclinic@nhs.net or posted to:
