Referrals & Staff Information

The clinic has a national referral base and has agreed referral pathways with the local CCGs. Referrals are accepted from primary, secondary and tertiary care. 

How To Make a Referral to the Cognitive Disorders Clinic

For general practitioners referring patients, referrals must be submitted via ERS (NHS e-referrals portal).
