You will need

• This completed form;
• your name;
• your department;
• your contact details (email and number);
• the dates that you require for storing;
• a brief description of items to be stored (send pictures if possible); and
• if iyour tems are large/heavy and require lift assistance, please provide their weight and dimensions.
