What caused the Whaley Bridge dam’s partial collapse?

Professor Tim Broyd (UCL Bartlett School of Construction & Project Management) explains that dams are well regulated structures, inspected by highly qualified engineers, and that any issues were likely caused by an extremely high flow rate into the reservoir after intense rain.
