Supporting Excellence in End of life care in Dementia (SEED)

Programme Summary

Our aim was to support providers and commissioners to identify and deliver good quality, community-based end of life care in dementia.

This programme grant consisted of 6 work streams and MCPCRD (Professor Elizabeth Sampson) led work stream 1: Mapping existing guidance/pathways and identification of outcome measures for end of life care in dementia. We summarised existing recommendations and examples of UK best practice including the quality of the evidence base underpinning them, identified and reviewed person-centred indicators/outcomes to measure good quality end of life care and identified of resource implications/benefits of existing pathways to inform subsequent modelling in WS5.
