UCLP-Primrose pathway

The UCLP-Primrose pathway combines Primrose-A (watch Population Health Nurse Anne-Marie Kay and Consultant Psychiatrist Dr Ian Prenelle introduce Primrose A here) with the UCLPartners Practice Care Frameworks (watch Programme Director and GP Dr Matt Kearney Introduce UCLPartners Proactive Care Frameworks here). Read about the history behind UCLP-Primrose in this blog: Supporting the whole person: Why physical health matters for patients with severe mental illness

Diagram showing the patient pathways for UCLP-Primrose including patient stratification, routes for patient engagement and non-engagement, outreach, and intensive behaviour change sessions.

You can view all the manuals and training that have been produced by UCLPartners here: UCLPartners UCLP-Primrose resources

These are comprehensive and have been designed to guide clinicians through the parts of the pathway they will be interacting with. For example, here is an a snapshot from the intensive behaviour change sessons manual for when clinicians are supporting patients to set goals:

Table to show clinicians clinicians the order of goals, from highest to lowest impact. Low blood pressure, lower cholesterol, stop smoking, manage diabetes, manage prediabetes, lose weight, and reduce alcohol intake.
