What are personalised care planning conversations and care plans?
This module explores what personalised care planning means in dementia care and why conversations are at the heart of good care planning. Drawing on policy, guidance, and the experiences of people with dementia, carers and practitioners, it develops a shared understanding of care planning conversations, care plans, and the communication skills that help people feel listened to, involved and supported.
This module currently contains:
- An introductory document exploring:
- what personalised care planning conversations are
- what care plans are and how they are used
- how important topics can be discussed without conversations feeling like a checklist
- the role of communication in supporting meaningful involvement
- how to balance the perspectives of people with dementia, carers and practitioners
Coming soon:
A professionally produced film featuring people with dementia and carers sharing their experiences of conversations with health and social care practitioners, including what helps them feel listened to, involved and supported.
