Research opportunity: 10 minute health questionnaire with a chance to win one of four £50 prizes

The aim of this research is to investigate the reasons why people engage in activities that affect their health, and how some people might be encouraged to adopt healthier lifestyles.
You will be asked a few questions on smoking, alcohol consumption, physical activity and fruit and vegetable consumption. Then you might also be provided with some information about one of these activities. The survey will last approximately 10 minutes.
If you complete the questionnaire you will be entered into a prize draw to win one of four £50 prizes.
To be able to participate you must be over the age of 18 and a UK resident.
