Many older adults, who are prescribed polypharmacy (>4 medicines) often have difficulties in taking medicines as prescribed, leading to non-adherence. This has been linked to poor clinical outcomes and increases in health utilisation costs. The SMAP intervention has been developed based on theory, recommended best practice and consultation with patients. It is designed to be delivered by community pharmacists.
The SMAP intervention has previously undergone feasibility testing and it is now being more extensively assessed in a pilot study in community pharmacies in Belfast and London.
