Applying for the UCL Cyclescheme
You will be required to provide details of your chosen bike including make, model and price of the equipment & accessories. You will also be asked to provide personal details such as your full name, address, telephone number, date of birth and email address.
If you are not comfortable providing such details please replace your personal details with your work details in the mandatory fields. You can use a default date of birth of 01/01/1901 if you do not wish to provide your actual date of birth. Although the employee payroll number field is not mandatory on this form failure to complete it is likely to cause a delay in processing your application.
