Departmental Exit Questionnaire

Information on the newly launched optional departmental exit questionnaire to support departments with additional information when individuals leave UCL.

UCL HR has launched a standardised and optional departmental exit questionnaire.  This new, additional questionnaire is designed to complement the central online exit survey that all employees who resign from UCL are asked to complete.  The questionnaire goes into more detail for departments wishing to analyse why staff are leaving their department and HR has worked with departments to capture the type of questions which they would find useful.

Departments are asked to ensure that any current departmental exit questionnaires are discontinued and that the centrally approved one is now adopted, departments can choose to use some or all of the questions provided.  Departments have flexibility on how the questionnaire is issued, how the responses are collated and can automate the form if they choose. Thought must be given to how and if anonymous responses may be submitted.   The department questionnaire must not be sent out until the week before the employee leaves so that they have first had the opportunity to complete the central survey.
