Consultations about self-management and diabetes

Background and aims
This study aims to investigate low uptake of diabetes structured education through research into the interactions of GPs and patients. Previous research suggests that many patients with type 2 diabetes do not engage with diabetes structured education. How GPs discuss diabetes education courses may be a barrier to patient attendance at these events. This study investigates whether and how GPs discuss self-management of diabetes, and diabetes structured education with patients during routine consultations, and how patients respond.
Research
This study looks at the practices employed by GPs to raise issues of self-management and structured education by using a technique called conversation analysis.
During this study we have examined two data sets:
- The HARI study conducted at UCL
- The One in a Million study conducted at University of Bristol
These studies provided archival video and audio footage of GP and patient interactions during GP consultations in routine practice. For the CASMAD study, the data sets were screened for consultations with patients with prediabetes and type 2 diabetes in which self-management was discussed.
16 transcripts in the data sets met the inclusion criteria. We are applying techniques of conversation analysis to the data to consider how actions are constructed and produced in interaction. By doing this we hope to learn how conversations around self-management take place, how GPs talk about self-management and self-management education, and how patients respond to these conversations. This will be important in helping GPs understand the best way to discuss self-management with patients to increase use of self-management resources.
Patient and Public Involvement (PPI)
The study was developed through conversations with a PPI group as part of the HeLP Diabetes [HT5] study, where the question of increasing uptake of all types of diabetes education arose.
Advice has been sought from a PPI representative on the development of the protocol and the representative will also be consulted on the final report. A further two PPI representatives will be recruited to attend a dissemination workshop.
Results
This study is ongoing. Once the data has been analysed we will produce a publication of the findings and produce recommendations for practice.
At the end of the study we will be hosting a dissemination workshop for healthcare professionals and people with type 2 diabetes to present the findings and discuss the recommendations for practice. If you are interested in attending thise event contact Jamie Ross.
