The aim of developing this toolkit was to enable personalised care for community-living people with Parkinson’s, to reduce disability and preventable hospital admissions and to improve quality of life. The toolkit was developed through co-production and tested in a feasibility study. The effectiveness is being evaluated through a Randomised Controlled Trial.
