Appointment Forms

Please use SiP for any new appontments or changes in appintments

Important Information for All Departments

The following paper based employment administration forms have been withdrawn from the HR website:

  • Form 1/2 - Appointment Forms
  • Form 1/2A - Change to Appointment Forms
  • Form 3 - UCL Teaching Fellow Appointment
  • Form 4/5 - Appointment of Casual Staff, Postgraduate Teaching Assistants and Demonstrators
  • Form 6 - Requests for Payment for Additional One Off Duties
  • Form 7 - One-Off Payments
  • Leavers Notifications (inclusive of retirement forms)
  • UCL Retirement Notification Form

HR and Finance will no longer accept paper based forms. With effect from June 1st 2006, departments must submit their employment administration instructions via the electronic workflow tool -SiP http://www.ucl.ac.uk/hr/sip/.

Departmental administrators who have still not registered to process their forms electronically must contact the following HR managers for guidance on how to sign up to use SiP :
