HSE GM Notification (for activity class 2, 3 and 4)
CU2 For notifying GM projects of activity class 2, 3 or 4 to the HSE. This version contains cross references to UCL GMM forms
RA In addition to the CU2 form, a full risk assessment must be submitted with the CU2 form . This suggested format is cross referenced to sections in the UCL GM notification forms
