SUPERVISOR’S FEEDBACK FORM AND TRAINEE FEEDBACK FORM
This section is a quick guide to the forms used by supervisors and trainees to evaluate placements.
All these forms are described in more detail in relevant sections of this handbook, and the forms themselves are downloadable from the Course website.
Forms used by supervisors
Giving feedback at the end of a placement
At the end of each placement supervisors use the standard Supervisors’ Feedback Form to give feedback to the Course and hence to the Exam Board. This provides comprehensive feedback about trainee progress and enables the Exam Board to pass or fail the trainee.
Giving feedback at the mid-point of a one-year placement
Technically one-year placements comprise two six-month placement periods, each of which need to be passed, and hence each of which need to be formally evaluated. The “supervisor's feedback form for use at the mid-point of a one year placement” is a briefer version of the standard feedback form, allowing the supervisor to indicate trainee progress to date.
Feedback form for supervisors who are not the main supervisor
This form allows for brief feedback from supervisors who have offered limited supervision to a trainee and who wish to add some separate feedback to that offered by the main supervisor. (For example, where they will have been offering some limited complementary supervision of one or two cases in a model different to that of the main supervisor).
Form for registering concern regarding trainee progression at a Mid-Placement Review
If serious concerns about trainee progress emerge at the Mid-Placement Review the MPR visitor will document these in the MPR report. However, the supervisor also needs formally to signal their concerns to the Course using this one-page form.
Interim feedback form for third year trainees nearing the end of the course
The main Examination Board (at which final decisions about passing or failing are made) meets in early September, slightly ahead of the point at which trainees complete the course (which is at the end of September). The Board needs a formal indication of trainee progress on placement, but at this point the standard supervisor form will not have been completed. To resolve this, the ‘interim’ form is a one-page form which simply asks supervisors to indicate whether the trainee is expected to pass the placement.
Form used by trainees
Trainees complete a Trainee Feedback Form which asks for feedback about their experience of the placement, the clinical work available and the supervision they received.
